Problems tend to become visible downstream from their cause.

A missed deadline appears to be an execution problem. It may begin with unclear priorities. Customer complaints look like a support issue. They may originate in pricing, incentives, or product design.

Treating the visible symptom can provide relief while preserving the system that keeps producing it.

I find it useful to ask a simple question: what has to be true earlier in the chain for this problem to keep recurring here?

The answer often moves responsibility to a less obvious place. That is uncomfortable, but it is where improvement begins.
